Maximal extractable value (MEV) is profit block builders and searchers make by reordering, inserting, or censoring transactions within a block — commonly via sandwich attacks around large DEX swaps. It's a structural feature of public mempools, not a bug, and is one reason large trades are often routed through private order flow or MEV-protected RPCs.
